
We support people in Blaenau Gwent who are living with a life limiting condition. The conditions can include cancer, heart failure, COPD, dementia and neurological conditions such as motor neuron disease and multiple sclerosis.
Our team is made up of experienced professionals who have all had additional training and an in-depth knowledge of palliative care. The team can provide support in a variety of settings which include your home, nursing or residential home, local hospitals including Ysbyty Aneurin Bevan or Ysbyty Tri Chwm and for those “Living Well” at our out-patient clinic at the Hospice of the Valleys building. Our team works closely together and alongside other health and social care providers including your GP, the district nurses, hospital teams and voluntary workers. (NEED TO CHECK CLINICAL DETAILS)
Our services can help you and those who matter most to you with physical, emotional, social and financial concerns. We are here to support you and your loved ones through some of life’s most challenging moments, guided always by what matters most.
We also have a designated service for patients living with Dementia and their carers called CARIAD.
You may only need support from Hospice of the Valleys for a short time and may be discharged from us if your symptoms or concerns are managed, but we’re always here for you.
