Our Community

We are the heart of hospice care in Blaenau Gwent.

We’re dedicated to helping people in our community to live and die well, while supporting those who matter most to them. We provide a holistic and high-quality service that is bespoke to each patient and their loved ones, ensuring they feel safe and supported throughout their hospice journey.

We support people in Blaenau Gwent who are living with a life-limiting condition. They can include cancer, heart failure, COPD, dementia and neurological conditions such as motor neuron disease and multiple sclerosis.

Many of our professional team live locally too, motivated by wanting to support their local community when needed most.

Our Team

Our team is made up of experienced professionals who have all had additional training and an in-depth knowledge of palliative care. The team can provide support in a variety of settings which include your home, nursing or residential home, local hospitals including Ysbyty Aneurin Bevan or Ysbyty Tri Chwm and for those “Living Well” at our out-patient clinic at Hospice of the Valleys.  Our team work closely together and alongside other health and social care providers including your GP, district nurses, hospital teams and voluntary workers.

Our support can help you and those who matter most to you with physical, emotional, social and financial concerns.

We can help you plan for the future. While providing support at the end of your life, we can also be there for your loved ones in their bereavement.

We also have a designated service for patients living with Dementia and their carers called CARIAD.
